What is disease management?
Disease management is a program designed to assist members suffering from asthma, depression, diabetes, congestive
heart failure and/or chronic obstructive pulmonary disease. The program is provided by TACT, your employer, and their
partner Q-elements, a national care management firm. The emphasis of the program is on the importance of providing our
members with the education and support initiatives to help them improve their health and quality of life.
Why should I be interested in the program?
The goal of the program is to provide improvement in health status and quality of life while managing health care
expenses. This is accomplished by providing education about chronic condition(s) and working with members on lifestyle
changes in conjunction with their treatment plan. This helps members better understand and take control of their
condition and proactively participate in their care and treatment.
Who pays for the program?
This valuable benefit is a part of your health care plan at no extra cost to you.
Does the program replace my physician?
The program does not replace physician-patient relationships. It is designed to complement the relationship and reinforce
the treatment plan of care established by you and your physician.
Will my personal health information remain confidential?
Your health information is highly confidential with information only disclosed as permitted or required by law.
How do I enroll?
Once a member is identified as having one of the chronic conditions listed above, they are automatically enrolled in the
program. The member and their physician receive a welcome letter from Q-elements confirming enrollment along with a
follow-up telephone call from a registered nurse. If you have any questions or want to enroll in the program, please
